Sevenoaks District Council are offering grants of up to £40,000 to charities, health and voluntary organisations to improve links between health and social care.
The Council wish to improve the way the housing related element of services is offered and integrate health services, social care, the voluntary sector and Sevenoaks District Council. Better integration between these services will reduce unnecessary demands for both primary and secondary care and improve the wellbeing of our residents. Sevenoaks District Better Care Grant is aimed at facilitating innovation in the way preventative and other projects help older people, people with long term conditions and people with disabilities stay well, manage their conditions better or recover from illness or injury.
In particular, the Council are looking for applications that will help increase applications for financial assistance available via the 2017 Housing Assistance Policy.
Deadline: Wednesday 28 February 2018. Applications will appraised on a rolling “first come – first served” basis, so early applications are advised.
